Food Protein-Induced Enterocolitis Syndrome

Food protein–induced enterocolitis syndrome (FPIES) is a non-immunoglobulin E (IgE)- mediated food allergy. However, the exact pathophysiology of FPIES is not clearly understood, cell mediated mechanisms are considered to be responsible. FPIES is classified in two forms: acute and chronic. There are no diagnostic laboratory tests for FPIES but laboratory methods can be used for other diseases in the differential diagnosis. The diagnosis of FPIES is based on clinical responses to elimination of suspected diets with resolution of symptoms, oral food challenges with reappearance of symptoms following provocation test by providing the offending food. Acute FPIES can be confused with other frequently seen childhood diseases. In this review, the clinical findings, diagnosis, diseases included in the differential diagnosis, treatments and follow-up of FPIES are examined in the light of the latest publications and recommendations.

Besin Proteini İlişkili Enterokolit Sendromu

Besin proteini ilişkili enterokolit sendromu (BPİES), immunglobulin E (IgE) aracılı olmayan besin alerjisidir. BPİES’in patofizyolojisi henüz tam aydınlatılamamakla birlikte, patofizyolojisinde hücre aracılı mekanizmaların sorumlu olduğu düşünülmektedir. Akut ve kronik olmak üzere iki şekilde sınıflandırılır. Tanı koydurucu laboratuvar testleri yoktur fakat ayırıcı tanıya giren diğer hastalıklar için laboratuvar yöntemlerinden yararlanılabilir. Tanı şüpheli besin diyetten çıkarıldıktan sonra yakınmaların tamamen düzelmesi ve besin provokasyonla yine verildiğinde akut BPİES bulgularının oluşması ile konulur. Akut BPİES sık görülen diğer çocukluk çağı hastalıkları ile karıştırılabilir. Bu derlemede, BPİES’in klinik bulguları, tanısı, ayırıcı tanıya giren hastalıkları, tedavisi ve izlemi son yayınlar ve öneriler eşliğinde incelenmiştir
