UROLOGIC COMPLICATIONS IN THE MANAGEMENT OF ABNORMAL PLACENTAL INVASION; 5-YEARS OF EXPERIENCE OF SINGLE TERTIARY CENTER

Objective: To evaluate the urologic considerations in the management of abnormal placental invasion in a tertiary care institution.

Materials and methods: This is a retrospective review of all patients with abnormal placental invasion delivered from March 2010 to October 2015 in İstanbul University School of Medicine, Department of Obstetrics and Gynecology. The patients with urologic injury were identified and evaluated according to details of operation.

Results: 72 patients were included in the analysis. The depth of abnormal placental invasion varied in the cohort, with 31.9%, 19.4% and 48.6% being classified as placenta acreata, placenta increata and placenta percreata, respectively. Bladder injury was encountered in 30.5% of patients and ureteral injury was seen in 2.8%. Degree of placental invasion were associated with a higher likelihood of urologic injury.

Conclusion: Urologic injuries are the most common intraoperative complication in the cases of abnormal placental invasion. Surgeons who involved in the management of these cases should be familiar with this risk and keep careful observation intraoperatively.

PLASENTA İNVAZYON ANOMALİLERİNİN YÖNETİMİNDE ÜROLOJİK KOMPLİKASYONLAR; TEK TERSİYER MERKEZİN 5 YILLIK SONUÇLARI

Amaç: Plasenta invazyon anomalileri vakalarında ürolojik komplikasyonlarının tersiyer merkez sonuçlarının değerlendirilmesi.

Gereç ve yöntem: Plasenta invazyon anomalisi tanısı olan Mart 2010 ve Ekim 2015 yılları arasında İstanbul Üniversitesi Kadın Hastalıkları ve Doğum Kliniği’ne başvurmuş tüm hastaların sonuçları retrospektif olarak değerlendirilmiştir. Ürolojik hasar oluşmuş hastalar tanımlanmış ve operatif detayları değerlendirilmiştir.

Bulgular: Çalışmada 72 hastanın verileri incelenmiştir. Hastaların %31.9’u plasenta akreata, % 19.4’ü plasenta inkreata, %48.6’sı plasenta perkreata tanısı almıştır. Mesane hasarı hastaların % 30.5’inde saptanırken, üreter hasarı ise hastaların %2.8’inde görülmüştür. Plasenta invazyon derecesi ile mesane hasarı arasında yüksek birliktelik saptanmıştır.

Sonuç: Ürolojik yaralanmalar plasental invazyon anomalisi vakalarında en sık karşılaşılan intraoperatif komplikasyonlar arasında yer almaktadır. Bu vakaların yönetimine dahil olan cerrahların bu risk konusunda haberdar olması ve operasyon sırasında dikkatlice gözlem yapılması gerekmektedir.
