COVID-19 and Breastmilk

Coronavirus 2019 (COVID-19) disease is a viral infectious disease caused by SARS-CoV-2, causing severe acute respiratory failure. The first case was reported in November 2019 and quickly caused a global pandemic which has led to many medical, social and economic consequences. One of the most important of these effects was the SARS-CoV-2 infection in the postpartum management of pregnant women, the contact of the mother and the babies, and breastfeeding. To date, there is no clear evidence of COVID-19’s vertical transmission from mother to baby. Although the transmission of viruses with breast milk from mothers with COVID-19 infection has not been reported, concerns remain about the transmission of the infection to the baby through respiratory droplets or during close mother-child contact. Breast milk, which has many benefits and unique features known to babies in the short and long term, has a very important role in protection against viral and bacterial infections. In breast milk, especially in colostrum, high secretory immunoglobulin (Ig)A, other specific immunoglobulins and some antiviral factors and bioactive substances, cellular components are present. Therefore, it is especially important to continue breastfeeding during an outbreak that affects the whole world. The aim of this review is to indicate the protective properties of breast milk from infections and to present the precautions that should be taken during breastfeeding in the babies of the mothers with COVID-19 infection in the light of current scientific data.

COVID-19 Hastalığı Pandemisi ve Anne Sütü

Koronavirüs 2019 (COVID-19) hastalığı, SARS-CoV-2’nin sebep olduğu viral bir enfeksiyon olup, ağır akut solunum yetmezliğine neden olmaktadır. İlk vaka Kasım 2019’da bildirilmiş olup hızla global bir pandemi (küresel salgın) oluşmuş hem medikal, hem sosyal hem de ekonomik pek çok sonuca yol açmıştır. Bu etkilerin en önemlilerinden biri de SARS-CoV-2 enfeksiyonunun gebelerin doğum sonrası yönetiminde, anne ile bebeklerin birbiriyle temasında ve anne sütüyle beslenmesi üzerine olmuştur. COVID-19’un anneden bebeğe vertikal geçişi ile ilgili şimdiye kadar kesin kanıt yoktur. COVID-19 enfeksiyonlu annelerden bebeklerine anne sütü ile virüs bulaştığı rapor edilmemiş olsa da enfeksiyonun solunum damlacıklarından veya yakın anne-çocuk teması ile bebeğe bulaşmasına ilişkin endişeler devam etmektedir. Bebeklere kısa ve uzun vadede bilinen çok sayıda faydası ve eşsiz özellikleri olan anne sütünün özellikle viral ve bakteryel enfeksiyonlara karşı korunmada çok önemli bir rolü vardır. Anne sütünde özellikle kolostrumda yüksek salgısal immünglobulin (Ig)A, özgün diğer immünglobulinler ve bazı antiviral faktörler ve biyoaktif maddeler, hücresel bileşenler bulunmaktadır. Daha da ötesi anne sütünde SARS-CoV-2’ye karşı antikorlar anektodal olgularda tespit edilmiştir. Bu nedenle tüm dünyayı etkileyen bir salgında anne sütüyle beslenmenin devam etmesi özellikle önemlidir. Bu derlemenin amacı anne sütünün enfeksiyonlardan koruyucu özelliklerini belirtmek; COVID-19 enfeksiyonu olan anne bebeklerinde anne sütü ile beslenme sırasında alınması gereken önlemleri mevcut bilimsel veriler ışığında sunmaktır.
