Urology Practice in the era of the COVID-19 Pandemic

The Coronavirus disease 2019 (COVID-19) pandemic has become the most serious health threat. The disease of COVID-19 is a viral infection caused by severe acute respiratory syndrome coronavirus-2 and causing severe acute respiratory failure. Medical and surgical treatments have changed during the process and started to be applicable in different ways. These changes include postponing visits and operations for elective patients and organizing work according to emergency cases and COVID-19 patients. In daily urology practice, emergency and unpostponable problems are only 5-10% of the total. Total obstruction of the urinary tract, serious trauma of the urinary tract, severe urinary tract infections and some advanced urinary tract cancers cannot be postponed. For these problems, medical procedures should be converted as soon as possible, with maximal protection procedures. This review summarizes the changes in the field of urology in accordance with the current literature. In this review, we provide guidance on the diagnosis and management of COVID-19, the influence of it on urological practice and consider the long-term implications that may be of consequence for years to come. There is a need for more well designed research against this horrible threat.

COVID-19 Pandemisinde Üroloji Uygulamaları

Koronavirus 2019 hastalığı (COVID-19) pandemisi (küresel salgını) son yıllardaki en ciddi sağlık tehdidi halini almıştır. COVID-19, şiddetli akut solunum sendromu koronavirus-2 (SARS–CoV–2)’nin sebep olduğu viral bir enfeksiyon olup, ağır akut solunum yetmezliğine neden olabilmektedir. Medikal ve cerrahi tedaviler bu süreçte dramatik olarak değişmiş ve farklı şekilde uygulanabilir hal almıştır. Bu değişiklikler seçili hastalar için ziyaretlerin ve operasyonlarının ertelenmesi, etkinliklerin acil vakalara ve COVID-19 hastalarına göre düzenlenmesini içermektedir. Bu süreçte yoğun bakım yatakları, ventilatörler, operasyon odaları, kan ve kan ürünlerinin stoklarının gözden geçirilmesi ve yeniden planlanması gerekmektedir. Çalışma saatlerinin ayarlanması, personel kapasitesinin gözden geçirilmesi, onam formlarının bu hastalığa göre düzenlenmesi gibi bir dizi hazırlık yapılmalıdır. Kişisel koruyucu ekipmanları (maske, önlük, eldiven vb) bütün personelin kullanması, kullanımının öğrenilmesi için uygun eğitimlerin verilmesi gerekmektedir. Mümkün olduğunca, acil ürolojik girişimler yapılmalı, onkolojik cerrahinin bile bir süre ötelenebileceği akılda tutulmalıdır. Ürolojik cerrahide mümkün olduğunca klasik en hızlı cerrahi metod uygulanmalı, laparoskopik cerrahi gibi bulaş riskinin arttığı işlemler tercih edilmemelidir. Radyoterapi, kemoterapi gibi ilave tedaviler de bu sürede en az zararlı şekilde düzenlenmelidir. İmmün sistemi etkileyebilecek tedavilerden uzak durulmalıdır. Bu derleme üroloji alanında COVID-19 küresel salgını esnasındaki değişiklikleri güncel literatüre uygun olarak özetlemektedir.
