Kroni̇k Böbrek Hastalığı Olan Yaşlılarda Peri̇operati̇f Yaklaşım

Yaşlı yetişkin nüfus (yaş >65 yıl), dünya nüfusunun en hızlı artan kesimidir. Günümüz dünyasında artan yaşam beklentisi ile birlikte, artık daha fazla yaşlı, daha fazla sayıda operasyona ihtiyaç duymaktadır. Ancak yaşlı hastaların, ameliyat streslerini veya komplikasyonlarını tolere etme kapasiteleri genç hastalara göre azalmıştır. Yaşlanmanın kendisi operatif riski minimal olarak artırırken yaşlanmayla birlikte kronik hastalıkların prevalansındaki artış ve organ fonksiyonlarında ilerleyici bozulma nedeniyle risk oldukça yüksektir. Yaşlanan nüfusta, artan komorbiditeler ve daha karmaşık cerrahi prosedürler ameliyattan sonra iyileşmeyi olumsuz etkilediği gibi yüksek riskli hasta sayısının artmasına da neden olur. Cerrahi operasyon geçirenlerin üçte biri 65 yaş üstündedir ve bu yaşlı kesimin %50’ sinde kronik böbrek hastalığı (KBH) bulunmaktadır. Ameliyat olacak KBH’ lı yaşlılarda detaylı bir değerlendirme ile perioperatif dönemde gelişebilecek istenmeyen olayları ve ölümü azaltmak mümkün olabilir.
