Pain Management/Analgesia, Anaesthesia and Ethics

Providing pain relief poses a unique ethical challenge compared to other medical treatments. Pain conditions should be assessed and treated without discrimination on the basis of race, gender, or economic status. Effective pain management is a moral obligation and a professional responsibility. Unethical practices in pain management can be prevented by providing education on ethical and legal responsibilities, developing skills for ethical sensitivity and supervising this in the clinic. Anaesthesia is a medical practice that prevents patients from feeling pain during surgery. General anaesthesia involves medical ethics issues as it initiates and manages a process in which the patient is completely unconscious. Patients undergoing surgery with regional anaesthesia are awake during the surgical procedure and are aware of the surgical procedure. The perceptions and experiences of patients undergoing surgery with regional anaesthesia regarding the care of the surgical process are different from those of patients undergoing general anaesthesia. Ethical issues that attract attention in the field of anaesthesia are reported as communication with patients and their relatives, interaction with the surgical team, long working hours and excessive workload, and problems in informing patients. Privacy in patients undergoing anaesthesia, unauthorised monitoring and video recording of surgery, and orders not to resuscitate are also included in ethical issues.

Ağrının giderilmesinin sağlanması, diğer tıbbi tedavilere göre benzersiz bir etik zorluk teşkil etmektedir. Bireylerin ırk, renk, cinsiyet veya ekonomik durumunda ayrım yapmaksızın, ağrı durumları değerlendirilmeli ve tedavi edilmelidir. Etkili ağrı yönetimi, ahlaki bir zorunluluk ve profesyonel bir sorumluluktur. Ağrı yönetiminde etik dışı uygulamalar, etik ve yasal sorumluluklara yönelik eğitimlerin sağlanması, etik duyarlılığa yönelik becerilerin geliştirilmesi ve klinikte bunun denetlenmesi ile önlenebilmektedir. Anestezi, hastaların cerrahi girişim sırasında ağrı hissetmesini engelleyen tıbbi bir uygulamadır. Genel anestezi, hastanın tamamen bilinçsiz olduğu bir süreci başlattığı ve yönettiği için tıp etiği konularını içermektedir. Bölgesel/lokal anestezi ile cerrahi geçiren hastalar, cerrahi girişim süresince uyanıktırlar ve girişimin farkındadırlar. Bölgesel/lokal anestezi ile cerrahi geçiren hastaların cerrahi süreçleri, bakıma ilişkin algıları ve yaşadıkları deneyimler genel anestezi uygulanan hastalardan farklıdır. Anestezi alanında dikkat çeken etik konular; hasta ve hasta yakınları ile iletişim, cerrahi ekiple etkileşim, uzun çalışma saatleri ve fazla iş yükü, hastaların bilgilendirilmesinde yaşanan sorunlar olarak bildirilmektedir. Anestezi uygulanan hastalarda mahremiyet, cerrahinin izinsiz izlenmesi ve video kayıtlarının alınması, resüsitasyon yapmama emirleri de etik konular içerisinde yer almaktadır.
