Esthetic Surgery and Ethics

The aim of aesthetic surgery is to reshape the external appearance of the body according to the perception of beauty in the mind of society or the individual. The principles of respect for autonomy, benefit, non-harm, and justice are emphasized in aesthetic surgery. In aesthetic surgery interventions, ethical problems can be seen in which surgical nurses have a responsibility toward their patients. Body image is becoming more and more important in society. In addition, the perception of beauty frequently imposed by the media can lead to ethical dilemmas in aesthetic surgery. From another perspective, the number of patients undergoing bariatric surgery is gradually increasing as a result of the increasing prevalence of obesity and the showing of bariatric surgery as a safe and effective way to treat patients diagnosed with obesity. Therefore, surgical nurses come in contact with ethical problems in the field of bariatric surgery. Surgery is a process in which the patient must give up his or her autonomy during the surgery. The establishment of an atmosphere of trust between the patient and the healthcare team and the right to informed consent is very important. Especially in malpractice cases, the patient’s informed consent must be obtained, and every intervention performed on the patient must be properly recorded. Since aesthetics is a subjective concept, surgical nurses have the responsibility to ensure that patients’ expectations are realistic. When deciding on surgery in aesthetic and bariatric surgery, the patient must be evaluated psychosocially. Surgical nurses should completely diagnose the medical and psychosocial anamnesis of patients planned for aesthetic surgery. Surgical nurses should be aware of mental disorders frequently seen in aesthetic surgery patients. In such cases, the indication for plastic surgery should be made in collaboration with a psychiatrist and a consultation liaison psychiatric nurse.

Estetik cerrahinin amacı, vücudun dış görünümünü toplumun ya da bireyin kendi zihnindeki güzellik algısına göre yeniden şekillendirmektir. Estetik cerrahide özellikle özerkliğe saygı, yarar sağlama, zarar vermeme ve adalet ilkeleri üzerinde durulmaktadır. Estetik cerrahi girişimlerde, cerrahi hemşirelerinin hastalarına karşı olan rol ve sorumluluklarında zaman zaman etik sorunlar görülebilmektedir. Günümüzde, toplumda beden imajı giderek çok daha önemli hale gelmektedir. Ayrıca medya tarafından sıklıkla dayatılan güzellik algısı da estetik cerrahide etik ikilemlere yol açabilmektedir. Başka bir açıdan, obezite prevalansının artması ve bariatrik cerrahinin obezite tanısı konan hastaları tedavi etmenin güvenli ve etkili bir yolu olarak gösterilmesi sonucu, bariatrik cerrahi uygulanan hasta sayısı giderek artmaktadır. Bu durum, cerrahi hemşirelerinin bariatrik cerrahi alanında da etik sorunlarla karşı karşıya kalmasına neden olmaktadır. Cerrahi süreç, hastanın cerrahi süresince özerkliğinden vazgeçmesi gereken bir süreçtir. Hasta ve sağlık ekibi arasında güven ortamının oluşması ve bilgilendirilmiş rıza hakkı bu nedenle çok önemlidir. Özellikle malpraktis (tıbbi hata) davalarında hastanın bilgilendirilmiş rızasının/onamının alınması ve hastaya yapılan her girişimin uygun şekilde kayıt edilmesi gerekmektedir. Estetik subjektif bir kavram olduğu için, cerrahi hemşirelerinin hastaların beklentilerinin gerçekçi olmasını sağlamaya yönelik sorumlulukları bulunmaktadır. Bu nedenle, estetik ve bariatrik cerrahi girişimlere karar verirken hastanın mutlaka psikososyal yönden değerlendirilmesi gerekmektedir. Cerrahi hemşirelerinin estetik cerrahi planlanan hastaların geçmiş tıbbi ve psikososyal öyküsünü alması ve tanılamasını eksiksiz yapması gerekmektedir. Cerrrahi hemşireleri, estetik cerrahi hastalarında sıklıkla görülen ruhsal bozuklukların farkında olmalıdır. Böyle durumlarda, estetik cerrahi endikasyonu bir psikiyatrist ve konsültasyon liyezon psikiyatri hemşiresi ile işbirliği içinde yapılmalıdır.
