Palliative Care and Ethics

Palliative care is essential to comprehensive care for all patients with chronic critical illness, including those receiving restorative or life-sustaining therapies. Core elements include alleviation of symptom distress, communication about care goals, alignment of treatment with the patient’s values and preferences, transitional planning, and family support. Palliative care performed as a holistic should be carried out within the framework of ethical rules. It is important to find what is in the patient’s best interest in the concept of palliative care and ethics. Understanding the situation of patients is important in shaping the roadmap in ethical dilemmas. Ethical rules are integrated into people’s care within the healthcare system. Within the framework of these ethical principles, palliative care patients should be given care that includes respect for their autonomy, not harming, benefiting, and fairness. Increasingly, people want to have more control over the final stage of their lives in making decisions about their medical treatment and care. In the terminal period, Advanced directives should guide life decisions. Nurses should help palliative care patients and their families make informed choices about health care, inform them by considering ethical principles in end-of-life decisions, prepare them for decision-making, and take on the role of patient advocacy. In this context, nurses should receive training on various rules and regulations and should reflect these regulations on patient care.

Palyati̇f Bakım ve Eti̇k

Palyatif bakım, kronik ve terminal dönemde hastalığı olan tüm hastalar için kapsamlı bakımın temel bir bileşenidir. Temel unsurlar arasında semptom sıkıntısının hafifletilmesi, bakım hedefleri hakkında iletişimin sağlanması, tedavinin hastanın değerleri ve tercihleri ile uyumlu hale getirilmesi, hasta bakımında esnek bir planlamanın yapıldığı ve ailenin desteğinin bakımda yer aldığı bir kavramdır. Bütüncül olarak yapılan palyatif bakım, etik kurallar çerçevesinde yürütülmelidir. Palyatif bakım ve etik kavramında, hastanın yararına olanı bulmak önemlidir. Hastaların içinde bulunduğu durumu anlamak etik ikilemlerde yol haritasının şekillendirilmesinde önem arz etmektedir. Sağlık bakım sistemi içinde, kişilerin bakımına entegre edilen etik kurallar vardır. Bu etik prensipler çerçevesinde palyatif bakım hastasına otonomisine saygı, zarar vermeme, yarar sağlama ve adalet ilkesini içeren bakım verilmelidir. Palyatif bakımda, kararların oluşturulmasında etik ikilemler gelişebilir. İnsanlar giderek artan bir şekilde yaşamlarının son evresi üzerinde tıbbi tedavi ve bakımları hakkında karar verme konusunda daha fazla kontrol sahibi olmak istemektedirler. Özellikle terminal dönemde ileri direktifler yaşamla ilgili kararların verilmesinde yol gösterici olmalıdır. Hemşireler palyatif bakım hastaları ve ailelerinin sağlık bakımı konusunda bilinçli seçimler yapmasına yardımcı olmalı, yaşam sonu ile ilgili kararlarda etik ilkeleri göz önüne alarak onları bilgilendirmeli, karar vermeleri için onları hazırlamalı ve hasta savunuculuğu rolünü üstlenmelidir. Bu bağlamda hemşire çeşitli kurallar, düzenlemeler hakkında eğitim almalı ve bu düzenlemeleri hasta bakımına yansıtmalıdırlar.
