Evaluation of Telehealth Follow-up Monitoring Data of Covid-19 Patients (Outpatient-İnpatient)

The use of telehealth services in the monitoring of COVID-19 disease reduces transmission by preventing physical contact. In this study it was aimed to examine the telehealth follow-up of patients diagnosed with COVID-19 in a university hospital, symptoms follow-up, and the factors affecting the persistence of their symptoms.With in the scope of this cohort study, the 28-days follow-up results of 759 patients between 15 March and 1 July 2020 were evaluated retrospectively. 59% of the follow-up patients received inpatient treatment and 41% received outpatient treatment. The most common persistent symptoms were malaise, cough, and dyspnea. As a result of multivariate logistic regression analysis performed with factors affecting symptom persistence, gender (OR=1.72 95%CI=1.19- 2.49), cardiovascular disease (OR=1.75 95%CI=1.04-2.94) ), COPD (OR=2.57 95%CI=1.26-5.25), and positive RT-PCR test result at diagnosis (OR=1.71 95%CI=1.19-2.47), were determined in relation to the persistence of COVID-19 symptoms. Symptoms may persist for a long time after treatment of COVID-19 disease. Informing patients about the persistence of their symptoms and associated factors may improve compliance with treatment and isolation. Ensuring that patients have Access to reliable information without going to the health institution is very important during the pandemic process which is possible with telehealth. The telehealth system makes a considerable contribution to the management of prolonged symptoms in COVID-19 disease.

Covıd-19 Olgularının (Ayaktan-Yatarak) Telesağlık İzlem Verilerinin Değerlendirilmesi

COVID-19 hastalığı izleminde telesağlık hizmetlerinin kullanımı fiziksel teması önlemesi sayesinde bulaşmayı azaltmaktadır. Bu çalışmada bir üniversite hastanesinde COVID-19 tanısı alan hastaların hekimler tarafından telesağlık ile yapılan izlemleri, semptom takibi ve semptomlarının devamına etki eden faktörleri incelemek amaçlanmıştır. Kohort tipindeki bu çalışma kapsamında, 15 Mart – 1 Temmuz 2020 tarihleri arasında, 759 hastanın 28 günlük izlem sonuçları retrospektif olarak değerlendirilmiştir. Takipteki kişilerin %55,9’u yatarak, %44,1’i ise ayaktan tedavi almıştır. En sık devam eden semptomlar halsizlik, öksürük ve dispne olmuştur. Semptom devamına etki eden faktörlerle yapılan multivariate lojistik regresyon analiz sonucunda cinsiyet (OR=1,72 95%CI=1,19-2,49), kardiyovasküler hastalık (OR=1,75 95%CI=1,04-2,94), KOAH (OR=2,57 95%CI=1,26-5,25) ve tanı anında pozitif RT-PCR test sonucu (OR=1,71 95%CI=1,19-2,47) son takipte COVID-19 semptom varlığı ile ilişkili olarak belirlenmiştir. COVID-19 hastalığı tedavisi sonrasında semptomları uzun dönem devam edebilmektedir. Hastaların semptomlarının devam edebileceği ve bununla ilişkili faktörler konusunda bilgilendirilmesi, tedavi ve izolasyona uyumu artırabilir. Hastaların sağlık kuruluşuna gitmeden güvenilir bilgiye ulaşmasının sağlanması, pandemi sürecinde oldukça önemlidir. Telesağlık sistemi COVID-19 hastalığında uzayan semptomların yönetiminde sürece önemli katkı sağlamaktadır.
