Telemedicine and Remote Health Management in Vascular Diseases

Telemedicine is the use of information and communication technologies by healthcare professionals in situations where distance is a critical factor for the diagnosis, treatment, and prevention of disease and injury, research and evaluation, and continuing education of healthcare providers. In 2020, the COVID-19 virus with crucial clinical manifestations, including death, emerged. It has been declared a pandemic because it threatens human health worldwide. The pandemic has various risk factors for patients who have chronic health problems. Reducing physical contact in this process has increased the interest in telemedicine applications. Both synchronous and asynchronous applications have been implemented. Legal infrastructure and financial issues regarding the provision of remote health services were taken into consideration. The Regulation on the Delivery of Distance Health Services in Turkey has been published. Systems and wearable technology products are needed for the delivery of remote health services. Both commercial products in the context of mobile health and products for biomedical purposes are being developed. These products include sensors for diagnosing vascular diseases such as Doppler ultrasound probe, oximeter, thermometer, dimention meter (for extremity circumference) and sphygmomanometer. The approach to the delivery of remote healthcare services moves from the development of sensors to system design. A holistic view emerges in the form of measuring with wearable technology products, entering the data into a system, and then evaluating it with machine learning. Systems are used for information, consultancy, diagnosis, and treatment purposes in vascular diseases. There are studies showing that telemedicine applications in this field increase patient satisfaction. However, telemedicine applications for vascular diseases are still at an early stage. There is a need for wearable technology products that can make all the necessary measurements for vascular diseases, system designs, and artificial intelligence applications that will add value to the data afterwards.

Damar Hastalıklarında Teletıp ve Uzaktan Sağlık Yönetimi

Teletıp, hastalık ve yaralanmaların teşhisi, tedavisi ve önlenmesi, araştırma ve değerlendirme, sağlık hizmeti sunanların sürekli eğitimi için mesafenin kritik faktör olduğu durumlarda bilgi ve iletişim teknolojilerinin sağlık profesyonelleri tarafından kullanılmasıdır. 2020 yılında ölüm dahil önemli klinik belirtileri olan COVID-19 virüsü ortaya çıkmış; dünya genelinde insan sağlığını tehdit etmesi nedeniyle pandemi ilan edilmiştir. Pandemi kronik sağlık sorunu bulunan hastalar için çeşitli risk unsurları barındırmaktadır. Bu süreçte fiziki temasın azaltılması teletıp uygulamalarına olan ilgiyi artırmıştır. Hem senkron hem de asenkron uygulamalar hayata geçirilmiştir. Uzaktan sağlık hizmetleri sunumuna dair yasal altyapı ve mali konular değerlendirmeye alınmıştır. Türkiye’de Uzaktan Sağlık Hizmetlerinin Sunumu Hakkında Yönetmelik yayınlanmıştır. Uzaktan sağlık hizmetlerinin sunumu için sistemlere ve giyilebilir teknoloji ürünlerine ihtiyaç vardır. Hem mobil sağlık bağlamında ticari ürünler hem de biyomedikal amaçlı ürünler geliştirilmektedir. Bu ürünler Doppler ultrason probu, oksimetre, ateş ölçer, boyut ölçer (ekstremite çevresi için) ve tansiyon ölçer gibi vasküler hastalıkların teşhisine yönelik sensörleri barındırmaktadır. Uzaktan sağlık hizmetlerinin sunumunda yaklaşım sensörlerin geliştirilmesinden sistem tasarımına doğru ilerlemektedir. Giyilebilir teknoloji ürünleri ile ölçüm yapılması, verilerin bir sisteme girilmesi ve sonrasında makine öğrenmesi ile değerlendirilmesi şeklinde bütüncül bir bakış sunmaktadır. Sistemler vasküler hastalıklarda bilgilendirme, danışmanlık, teşhis ve tedavi amaçlarıyla kullanılmaktadır. Bu alandaki teletıp uygulamalarının hasta memnuniyetini artırdığına dair araştırmalar vardır. Ancak vasküler hastalıklar için teletıp uygulamaları henüz erken aşamadadır. Vasküler hastalıklar için gerekli ölçümlerin tamamını yapabilecek giyilebilir teknoloji ürünlerine, sistem tasarımlarına ve sonrasında veriye değer katacak yapay zekâ uygulamalarına ihtiyaç vardır.
