Hyperamilasemia Determined During the Course of Ebstein-Barr Virus Infection

Macroenzymes are high molecular weight enzymes which could be in the serum in physiologic or pathologic conditions. The first described and the most frequently seen macroenzyme is macroamylase. Here we present a child with macroamylasemia. A 3-year-old boy was brought to the hospital with fever and cervical swelling. On his examination, multiple number of cervical lymphadenopathies, hepatomegaly and splenomegaly were detected. Acute Ebstein-Barr virus (EBV) infection was considered in the patient who had fever, cervical lymphadenopathies, hepatomegaly, and splenomegaly and atypical lymphocytes on the peripheral smear. On laboratory findings, amylase levels were elevated which was thought to be a secondary complication, like parotitis, secondary to EBV infection. During follow-ups of the patient monitored without any treatment, complaints and all findings regressed, while amylase levels coursed at high levels. Salivary gland ultrasound (US) performed to find the cause of elevated amylase levels, findings which might be secondary to viral infection were seen. On the abdominal USG, pancreas was normal but minimal splenomegaly was detected. Fractional clearance of amylase was determined as <%1, and the patient. After all findings, as a conclusion, elevated amylase levels on our patient was thought to be related to macroamylasemia rather than EBV infection. High amylase level could face us as incidentally detected laboratory abnormality. Recognizing macroamylasemia could prevent unnecessary examinations and interventions performed on the patients.

Akut Ebstein-Barr Virüs Enfeksiyonu Sırasında Saptanan Hiperamilazemi

Makroenzimler, fizyolojik ya da patolojik durumlarda serumda bulunan yüksek moleküler ağırlıklı enzimlerdir. Makroamilaz, ilk tanımlanmış olan ve en sık görülen makroenzimdir. Burada makroamilazemi bir çocuk hasta sunulmuştur. Üç yaşında erkek hasta ateş ve boyunda şişlik nedeniyle getirildi. Muayenesinde servikalde çok sayıda lenfadenopati, hepatomegali ve splenomegali saptandı. Ateşi, servikal lenfadenopati, hepatomegali, splenomegalisi olan ve periferik yaymasında atipik lenfositleri saptanan hastada akut Ebstein-Barr virüs enfeksiyonu düşünüldü. Laboratuvar tetkiklerinde amilaz düzeyi yüksek saptandı ve EBV enfeksiyonuna sekonder parotit gibi bir komplikasyon olabileceği düşünüldü. Tedavisiz izlenen hastanın takiplerinde yakınmaları ve muayene bulguları gerilemesine rağmen, amilaz düzeyinin yüksek seyrettiği görüldü. Amilaz yüksekliği nedeniyle yapılan tükürük bezi ultrasonografisinde (USG) viral enfeksiyona sekonder olabilecek bulgular olduğu görüldü. Yapılan abdominal USG’de pankreas normal ve minimal splenomegali saptandı. Bakılan fraksiyone amilaz klirensi <%1 saptanan hastada amilaz yüksekliğinin geçirilen enfeksiyon ile ilişkili olmaktan çok makroamilazemiye bağlı olduğu kanısına varıldı. Amilaz yüksekliği tesadüfen bulunmuş bir laboratuvar bozukluk olarak karşımıza çıkabilir. Makroamilazeminin tanınması hastalara gereksiz inceleme ve girişim yapılmasını önleyebilir.
