A Case of Central Nervous System Tuberculosis Diagnosed by Biopsy

Objective: Central nervous system tuberculosis is the most serious form of tuberculosis which can be mortal unless treated effectively. From clinical, and radiological aspects, it can be confused with several forms of infectious and noninfectious diseases. Surgical intervention may be necessary for definitive diagnosis in selected cases. Case: A 6- year- old child with a history of herpes encephalitis, was brought to our clinic with headache, and vomiting persisting for the last 15 days, and manifestations of somnolence for the last few days Physical examination and initial laboratory evaluation were within normal limits. Cranial MRI revealed a hypodense lesion with peripheral edema which caused shifting on the right temporoparietooccipital lobe. Necrotizing granulomatous inflammation and acid-resistant, stain- positive bacilli were identified during microscopic examination of the biopsy material retrieved from the lesion. Antituberculostatic therapy and corticosteroid treatment were initiated. After 18 months of antituberculostatic treatment, neuromotor development of the child progressed normally relative to his age. Conclusion: In cases when differential diagnosis is impossible with the aid of clinical and radiological findings-as is the case with our patient-surgical intervention may be obligatory.

Biyopsi ile Tanı Konulan Bir Merkezi Sinir Sistem Tüberkülozu Vakası

Amaç: Merkezi sinir sistemi (MSS) tüberkülozu, tüberkülozun en ciddi formu olup, etkili biçimde tedavi edilmediğinde ölümcül olabilir. Klinik ve radyolojik olarak pek çok enfeksiyöz ve enfeksiyon dışı hastalıkla karışabilmektedir. Ayırıcı tanıda, gerekli vakalarda kesin ayrım için cerrahi girişim gerekebilir. Vaka: Bir yıl önce herpes ensefaliti tedavisi alan altı yaşında erkek hasta, kliniğimize on beş gündür devam eden baş ağrısı, kusma ve son birkaç gündür eklenen uyuklama yakınması ile getirildi. Fizik muayenesinde ve ilk basamak laboratuvar incelemelerinde özellik saptanmayan hastanın beyin manyetik rezonans (MR) incelemesinde sağ temporoparitooksipital bölgede şifte yol açan, çevresi ödemli, hipodens alan mevcuttu. Lezyondan alınan biyopsi materyalinde nekrotizan granülomatöz iltihap ve aside dirençli bakteriler saptanması üzerine hastaya antitüberküloz ve kortikosteroid tedavi başlandı. Tedavisi 18 aya tamamlanan hastanın nöromotor gelişimi yaşına uygun seyretti. Sonuç: Klinik ve radyolojik bulgularla intrakraniyal yer kaplayıcı lezyonun ayırıcı tanısının yapılamadığı durumlarda, vakamızda olduğu gibi cerrahi yöntemlerle bir an önce kesin tanıya varılmalıdır.
