A Case of Miliary Tuberculosis Together With Rarely Localized Osteomyelitis With Cranial Involvement

Tuberculosis is a significant health problem in developing countries. The diagnosis of tuberculosis is quite difficult. The rate of confirmation by the culture which is the gold standard for the diagnosis is low. It is usually diagnosed clinically, radiologically and histopathologically. A 16-year-old young boy who was referred to our clinic, and diagnosed as bone and joint tuberculosis. While searching for granulomatous diseases his biopsy result was documented as necrotising granulomatosis in an external health center where the patient had resorted to and complained about the ache and swelling on his left foot. In this patient, miliary tuberculosis and involvement of central nerve system accompanied by bone and joint tuberculosis was detected. We aimed to emphasize the importance of remembering tuberculosis as an etiology of long-term soft tissue swelling and ache in our country.

Ender Yerleşimli Tüberküloz Osteomyeliti ve Kranial Tutulumu Olan Millier Tüberküloz Vakası

Tüberküloz gelişmekte olan ülkelerin önemli bir sağlık sorunudur. Tüberküloz tanısı çocuklarda oldukça güçtür. Tanıda altın standart olan kültürle doğrulama oranı düşüktür. Genellikle klinik, radyolojik ve histopatolojik olarak tanısı konur. Sol ayak sırtında şişlik ve ağrı yakınmaları ile başvurduğu dış merkezde lezyon yerinden yapılan biyopsi sonucunun, nekrotizan granülomatöz inflamasyon ile uyumlu gelmesi üzerine kliniğimize yönlendirilen 16 yaşında erkek hasta granülomatöz hastalıklar açısından tetkik edilerek kemik eklem tüberkülozu tanısı almıştır. Hastada kemik eklem tüberkülozuna MSS tutulumu ve millier tüberküloz da eşlik etmekte idi. Bu vaka ile eklem ve yumuşak dokuda oluşan uzun süreli ağrı ve şişliklerin etiyolojisinde ülkemiz için tüberkülozun da düşünülmesinin önemini vurgulamak istedik.
