Ecthyma Gangrenosum Related to Pseudomonas aeruginosa Infection Developed in a Child with Fanconi Aplastic Anemia

Objective: Ecthyma gangrenosum (EG), is a rare cutaneous lesion associated with Pseudomonas aeruginosa bacteremia. Herein, we present a pediatric EG case admitted without bacteremia. Case report: A 15-year old girl diagnosed with Fanconi aplastic anemia presented with newly occured vesiculopustular lesion suddenly developed on anterior aspect of her right knee and fever. She was in a good medical condition with fever of 38.5°C. There was a vesiculopustular lesion with a diameter of 3x3 cm with a black-colored center on her right knee surrounded by edematous, erythematous, and painful area. Laboratory examination revealed an increase in acute phase reactants. Blood and sterile aspiration cultures were performed, and she was started on teicoplanin plus piperacillin-tazobactam. For her neutropenia filgrastim (10 mcg/kg/day for 3 days) treatment was initiated. Aspiration culture from the lesion yielded Pseudomonas aeruginosa growth while blood culture remained sterile. Prominent regression was observed in the lesion with medical treatment. Conclusion: EG should be evaluated as a life-threatening emergency and should be treated urgently even in the absence of bacteremia.

Fankoni Aplastik Anemisi Tanılı Çocuk Hastada Pseudomonas aeruginosa Enfeksiyonuna Bağlı Gelişen Ektima Gangrenozum

Amaç: Ektima gangrenozum (EG), Pseudomonas aeruginosa bakteriyemisi ile ilişkilendirilen ender bir cilt lezyonudur. Burada, Fankoni aplastik anemi tanılı, bakteriyemi olmadan EG ile başvuran bir çocuk vaka sunulmuştur. Vaka sunumu: Fankoni aplastik anemisi tanılı 15 yaşında kız hasta, sağ diz ön yüzünde ani gelişen vezikülopüstüler lezyon ve ateş yakınması ile başvurdu. Genel durumu iyi olan hastanın ateşi 38,5°C idi. Sağ diz ön yüzde, 3x3 cm boyutlarında ortası siyah renkli, vezikülo-püstüler görünümde, çevresi eritemli, ödemli ve ağrılı lezyon mevcuttu. Laboratuvar incelemesinde akut faz yüksekliği saptanan hastaya kan kültürü ve dizinde bulunan lezyondan steril şartlarda örnek alınarak teikoplanin ve piperasilin-tazobaktam başlandı. Nötropeni açısından 10 mcg/kg/gün dozdan 3 günlük filgrastim tedavisi uygulandı. Lezyondan alınan aspirat kültüründe Pseudomonas aeruginosa üremesi olan hastanın kan kültürü steril sonuçlandı. Medikal tedavisine devam edilen hastanın lezyonunda belirgin regresyon izlendi. Sonuç: Ektima gangrenozum bakteriyemi olmasa da hayati tehdit edebilecek ciddi bir enfeksiyonun bulgusu olarak değerlendirilmeli ve uygun tedaviye hızlıca başlanmalıdır
