Evaluation of Epidemiological, Laboratory Measurements, Clinical and Prognostic Features, Treatments and Echocardiographies of Our Patients with Kawasaki Disease

Aim: Kawasaki disease is a multisytemic disease characterized by fever, rash, cervical lymphadenopathy, conjunctival congestion, and oral cavity and hand-feet changes, that occurs mainly under 5 years of age. The most common complication is coronary artey aneurysm. In our study, we present children with Kawasaki disease and compare with the other patients that are reported. Material and Method: This is retrospective study of 34 children with diagnosis of Kawasaki based with Eular/ Press criteria, We evaluated epidemiological, laboratory measurements, clinical and prognostic features, treatments and echocardiographies. Results: Cases ranged between 1-12 years with mean 7 years. Most of them were male (62.5 %), 37.5 % of female and 63.3 % of them were under five years. Most of the referrals were in January and least were in December. In one patient, the disease was related to Leptospira infection. Fever was present in all patients. The second most common sign was oral mucosal changes present in 80 % of cases. Hand-feet changes, cervical lymphadenopathy, bulbar conjunctivitis were present in decreasing frequency. Coronary artey aneurysm was observed in seven of the cases. Myocardial infarction and death were not seen. Conclusion: With early diagnosis and treatment, none of our patients devoleped serious complications

Kawasaki Hastalığı Tanısı Almış Hastalarımızda Epidemiyolojik, Klinik, Laboratuvar, Prognostik Özelliklerinin ve Ekokardiyografik Bulgularının Değerlendirilmesi

Amaç: Kawasaki hastalığı, genellikle beş yaş altında sık görülen ateş, döküntü, servikal lenfadenopati, eksüdatif olmayan konjunktival konjesyon, ağız içi ve el-ayak değişiklikleri ile karakterize bir multisistemik hastalıktır. En önemli komplikasyonu koroner arter anevrizmasıdır. Çalışmamızda, Kawasaki hastalığı olan çocuklar sunulmakta ve diğer vaka serileri ile kıyaslanmaktadır. Gereç ve Yöntem: Bu çalışma EULAR/PRESS kriterlerine göre Kawasaki hastalığı tanısı almış 34 çocuğun geriye dönük olarak incelenmesidir. Hastalarımızda klinik, laboratuvar, prognostik özellikler ve ekokardiyografik bulgular değerlendirildi. Bulgular: Başlangıç yaşı 1-12 yıl (ortalama 7 yıl) arasındaydı. Hastaların % 62.5’i erkek, % 37.5 kız ve % 63.3’ü beş yaş altındaydı. En çok Ocak ayında, en az Aralık ayında başvuru oldu. Bir hastamızda Leptospira infeksiyonu Kawasaki hastalığı başlangıcı ile ilişkili bulundu. Bütün hastalarda ateş saptandı. İkinci en sık görülen bulgu oral mukoza değişiklikleriydi ve hastaların % 80’inde vardı. El-ayak değişiklikleri, servikal lenfadenopati, konjunktival konjesyon azalan sıklıkla saptandı. Yedi vakada koroner arter anevrizması gözlendi. Hastalarımızın hiçbirinde miyokard infarktüsü ve ölüm görülmedi. Sonuç: Hastalarımızın hiçbirinde Kawasaki hastalığına bağlı ciddi komplikasyon gelişmediği görüldü. Bu durumun erken tanı ve tedaviye bağlı olduğu kanısına varıldı.
