The Evaluation of Urinary Incontinence in Preschool Age Children

Introduction: The urinary incontinence can be classified as organic and functional in children. In the etiology of the functional voiding disorders such as urge incontinence, lazy bladder and dysfunctional voiding several theories have been proposed. However, the exact etiology or pathogenetic mechanisms of the functionel urinary incontinence are not completely understood. It can be considered that toilet training or life style may be important in the etioloy of the urinary incontinence in preschool children. The aim of this study was to evaluate the voiding features of the children aged between 5-6 years. Materials and Methods: Between May 2010 and August 2010 a total of 271 children aged between 5-6 years were included to the study. The approval of the hospital ethics committee and written informed consent were obtained. Children were evaluated with dysfunctional voiding and incontinence symptoms score questionnaire. Children with neurogenic bladder, vesicoureteral reflux, metabolic disorder, history of recurrent urinary tract infection were excluded from the study. Results: Urinary incontinence was detected in 107 children (39.5%). The mean age of the children with or without urinary incontinence was 5.81±0.39 year and 5.88±0.33 years, respectively (p=0.142). There were no statistically significant difference in children with or without urinary incontinence in terms of body mass index, paternal or maternal age or duration of breastfeeding (p>0.05) (Table 1). The quality of life was affected negatively in 69 children (64.5%) with urinary incontinence. In contrast the quality of life was affected negatively only in 25 children (15.2%) without urinary incontinence. The quality of life was 12.05 fold worse in children with urinary incontinence than in children without urinary incontinence. Conclusion: Although in this study, it is revealed that urinary incontinence rates were relatively higher compared to literature in preschool age children studies in larger, randomised and controlled series are needed to confirm these results.

Okul Öncesi Çocuklarda İdrar Kaçırmanın Değerlendirilmesi

Amaç: Çocuklarda üriner inkontinans organik ve fonksiyonel olarak sınıflandırılabilir. Sıkışma inkontinansı, tembel mesane ve disfonksiyonel işeme gibi fonksiyonel işeme bozukluklarının etiyolojisinde pek çok teori ileri sürülmüştür. Bununla birlikte fonksiyonel üriner inkontinansın etiyolojisi tam olarak anlaşılamamıştır. Tuvalet eğitimi ve yaşam biçiminin okul öncesi çocuklarında idrar kaçırma etiyolojisinde rol oynayabileceği düşünülebilir. Bu çalışmanın amacı 5-6 yaş grubu çocuklardaki işeme özelliklerinin değerlendirilmesidir. Gereç ve Yöntem: Mayıs 2010 ve Ağustos 2010 tarihleri arasında yaşları 5 ile 6 arasında olan toplam 271 çocuk çalışmaya alındı. Etik Kurul onayı ile bilgilendirilmiş onam formu alındı. Çocuklar işeme bozuklukları semptom skoru formu ile değerlendirildiler. Sık idrar yolu infeksiyonu, vezikoüreteral reflü ve nörojen mesane öyküsü olan çocuklar çalışma dışı bırakıldı. Bulgular: Üriner inkontinans 107 (39.5%) çocukta tespit edildi.. İdrar kaçırması olan ve olmayan çocukların ortalama yaşları sırası ile 5.81±0.39 yıl ve 5.88±0.33 yıl olarak tespit edildi (p=0.142). İdrar kaçırması olan çocuklar ile idrar kaçırması olmayan çocuklar arasında boy, vücut kitle indeksi (VKİ), anne ve baba yaşı ile anne sütü alma süresi açısından istatistiksel olarak anlamlı fark tespit edilmedi (p>0.05). İdrar kaçırması olan çocukların %64.5’inde (n=69) yaşam kalitesi olumsuz olarak etkilenmişti. Buna karşın idrar kaçırması olmayanların yalnızca % 15.2’sinde (n=25) hayat kalitesi etkilenmişti. Buna göre idrar kaçırma yakınması olan çocukların yaşam kalitesinin idrar kaçırma yakınması olmayan çocuklara göre 12.05 kat daha kötü olduğu tespit edildi. Sonuçlar: Her ne kadar bu çalışmada idrar kaçırma oranlarının okul öncesi çağdaki çocuklarda literatüre göre nispeten daha yüksek olduğu ortaya konulsa da bu konuda geniş vaka sayısına sahip, kontrollü ve randomize çalışmaların yapılmasına gerek olduğu söylenebilir
