Acute Dystonic Reaction with Metoclopramide in a 12 Year Old Child

Nausea and vomiting are frequent complaints and can ocur in the course of several acute or chronic conditions. Antiemetic drugs are widely prescribed in general medicine

and pediatric practice for these types of complaints. Metoclopramide is the most common antiemetic drug that has been used for several years and it has become regarded

as a safe drug. However, metoclopramide is a dopamine antagonist and associated with the occurence of psychotic and extrapyramidal adverse effects. Since movement disorders

and psychotic reactions are rarely seen in pediatric population compared to the adult population, it can be difficult to diagnose them. This case report presents a 12

year old boy representing acute dystonic reaction 2 hours after receiving a single 5 mg dose of metoclopramide.

On İki Yaşındaki Bir Çocukta Metoklopramid Kullanımına Bağlı Gelişen Akut Distonik Reaksiyon

Bulantı ve kusma çok sayıda akut veya kronik sağlık sorunu süresince karşılaşılan yakınmalardır. Antiemetik ilaçlar bu tür yakınmalar için erişkin ve pediatrik hastalarda yaygın

olarak kullanılmaktadır. Metoklopramid de uzun yıllar boyunca en sık kullanılan antiemetik ilaç olmuştur ve bu süre boyunca güvenli olduğu düşünülmüştür. Ancak, dopaminerjik

antagonist olması nedeniyle, metoklopramid, psikotik ve ekstrapiramidal yan etkilerin gözlenmesi ile ilişkilendirilmektedir. Hareket bozuklukları ve psikotik reaksiyonların

pediatrik popülasyonda yetişkin popülasyona oranla daha ender olarak görülmesi nedeniyle teşhis edilmeleri güç olabilir. Vakamızda, 12 yaşındaki bir çocukta 5

mg dozluk metoklopramid kullanımından iki saat sonra gelişen akut distonik reaksiyon sunulmuştur.
