INVERTED PAPILLOMA: A COMPREHENSIVE CLINICAL ANALYSIS

Objective: This study aimed to present the clinical and pathological features that effect the recurrence and malignancy by analysis of results of inverted papilloma surgery performed in our center.

Materials and Methods: The data of 52 patients whom underwent surgery because of inverted papilloma at Department of Otorhinolaryngology, Istanbul Faculty of Medicine between 2008 and 2015 was investigated retrospectively. Demographic datas of patients, prior surgery, origin of tumor, stage, initial symptoms, and surgical techniques were evaluated.

Results: The mean age of 52 patients whom were treated from inverted papilloma was 59.3±12.4, and age range was 32-84. 41 of 52 patients were male and 11 were female. The distribution of the patients according to Krouse Staging system was determined such as; 1 of patients was T4, 4 were T2, 41 patients were T3 and 6 were T4.

In all, nine patients were found having squamous cell carcinoma or dysplasia. It is found that the smoking did not have an effect on recurrence (p=0.289) and prior surgery (p=0.748), prior surgery history was related to multiple location (p=0.043).

The mean follow-up period was 49,5±26,6 and minimum and maximum follow-up periods were 2-96 months. The mean age recurrence time was found as 41.3 months.

Conclusion: The most important factor to prevent the recurrence of inverted papilloma is to determine the location and to excise exactly. The relation of smoking and old surgery story with the reccurence could not be proved but it is seen that multiple location was related with repeating surgery. Larger case series and controlled studies are needed to obtain clearer statistical results.

İNVERTED PAPİLLOM: KAPSAMLI KLİNİK ANALİZ

Amaç: Çalışmanın amacı kliniğimizde uygulanan sinonazal inverted papillom cerrahisinin sonuçlarını analiz ederek rekürrens ve malignite üzerine etki eden klinik ve patolojik faktörleri ortaya koymaktır.

Gereç ve yöntem: 2008-2015 yılları arasında İstanbul Tıp Fakültesi KBB Kliniğinde inverted papillom nedeniyle cerrahi tedavi uygulanan 52 hastanın sonuçları retrospektif olarak incelendi. Hastaların demografik verileri, eski cerrahi, kaynaklandığı anatomik yer, evre, başvuru semptomları ve cerrahi teknik bilgisi değerlendirildi.

Bulgular: İnverted papillom tanısı ile tedavi edilen 52 hastanın yaş ortalaması 59.3±12.4 olup yaş aralığı 32-84’tür. 52 hastanın 41’i erkek, 11’i kadındı. Hastaların Krouse evreleme sistemine göre 1’i T1, 4’ü T2, 41’i T3, 6’sı ise T4 olarak dağıldığı saptanmıştır.

 Toplamda 9 hastada yassı hücreli kanser veya displazi olduğu saptanmıştır. Sigaranın nüks (p=0.289) ve geçirilmiş eski cerrahi (p=0.748) üzerine etkisi olmadığı, eski cerrahi öyküsünün multiple yerleşimle (p=0.043) anlamlı olarak ilişkili olduğu saptanmıştır.

Ortalama takip süresi 49.5±26.6 iken; minimum-maksimum takip süresi 2-96 ay idi. Ortalama nüks süresi 41.3 ay olarak bulundu.

Sonuç: İnverted papillomun nüksünü önlemede en önemli faktör yerleşim yerinin tespit edilmesi ve tam olarak eksize edilmesidir. Sigara ve eski cerrahi öyküsünün rekürrensle ilişkisi gösterilememekle birlikte, multiple yerleşimin tekrarlayan cerrahi ile ilişkili olduğu görülmektedir. Daha net istatistiksel sonuçların elde edilebilmesi için daha geniş vaka serileri ve kontrollü çalışmalara ihtiyaç vardır.
