AN EVALUATION OF TWO CASES OF POST TRAUMATIC HORNER SYNDROME WITHIN THE SCOPE OF TURKISH PENAL LAW AND DISABILITY REGULATIONS

HS is important in forensic medicine applications because it creates a permanent facial involvement (PFI) that disrupts the symmetry of face. This evaluation aims to discuss the PFI determinations and disability regulations in two cases where HS was found in the examinations of the 2nd Specialisation Board of the Forensic Medicine Institute. The first case was a 21-year-old female patient who had ptosis and anisocoria. Anterior cervical stabilization materials saw C6- T1 levels due to C7 compression fracture. The second case was a 36-year-old female patient with ptosis and myosis on the left. It was determined that there were fractures in the C7 left transverse processes and the T12-L1 right transverse processes and fractures in the left 1st, 2nd and 6th ribs. HS is an important facial involvement that can occur without direct trauma to the face or head. Our first case was evaluated as being secondary to the sympathetic involvement between C6-T1 levels, and the second case to the thoracocervical trauma accompanied by the left thoracic first rib fracture. The first of the cases was not evaluated as PFI. The second case was evaluated as PFI and has a 9% disability rate.
TRAVMA SONRASI HORNER SENDROMU GELİŞEN İKİ OLGUNUN TÜRK CEZA KANUNU VE MALULİYET YÖNETMELİKLERİ KAPSAMINDA DEĞERLENDİRİLMESİ
Nihade Hande Akçakaya, Medyar Koçak, Kağan Gürpınar, Hızır AslıyüksekHorner sendromu (HS) miyozis, pitozis, enoftalmus ve anhidrozis ile karakterize nörolojik bir tutulumdur. Vasküler, tümöral, cerrahi ve travmatik nedenlere bağlı ortaya çıkabilir. HS simetriyi bozan, yüzde sabit iz (YSİ) niteliğinde bir tutulum yarattığı için adli tıp uygulamalarında önemlidir. Adli Tıp Kurumu 2.İhtisas Kurulu muayenelerinde HS tespit edilen iki olguda, hem YSİ tayini hem de maluliyet yönetmeliklerindeki karşılıklarının tartışılması hedeflenmiştir. İlk olgu yirmi bir yaşında araç içi trafik kazası (AİTK) geçiren kadın hastaydı. Solda pitozis, anizokori mevcuttu. C7 çökme kırığı sebebiyle C6-T1 seviyeleri arasına anterior servikal stabilizasyon yapılmıştı. İkinci olgu otuz sekiz yaşında AİTK geçirmiş kadın hastaydı. Muayenesinde solda pupilla ortası pitozis ve miyozis saptandı. Olay tarihli servikal tomografilerde C7 sol transvers proçeste ve T12-L1 sağ transvers proçeslerde kırıklar, sol 1, 2 ve 6. kostalarda kırıklar ve sağ akciğer üst ve alt lob posterior kesimlerde parankimal kontüzyon mevcuttu. Yüz ya da baş bölgesine direkt travma olmaksızın oluşan HS tespiti kolay ancak gözden kaçabilecek bulgudur. İlk olgumuz C6- T1 seviyeleri arasında sempatik yol tutulumuna, ikinci olgu ise sol torakal birinci kot kırığının eşlik ettiği torakoservikal travmaya sekonder HS değerlendirildi. Olgulardan ilki YSİ olarak değerlendirilmemiştir. İkinci olgu YSİ ve %9 oranında maluliyet almıştır
