Neonatal Supraventricular Tachycardia: Diagnosis, Treatment and Factors Affecting Prognosis

Objective: Supraventricular tachycardia (SVT) is the most frequently encountered symptomatic tachyarrythmia in the neonatal period. Since it may onset during fetal life and show a severe clinical course, early diagnosis and immediate treatment is important. Material and Method: Clinical signs and symptoms, laboratory findings, accompanying diseases, treatment methods applied, responses to treatment and factors that affect treatment response in newborns that received the diagnosis of SVT in the neonatal intensive care unit of our hospital between September 2009 and August 2013 were retrospectively evaluated. Results: Median postnatal age of 17 infants that received the diagnosis of SVT was 16 hours (2-625), and 9 infants (52,9%) had congestive heart failure at admission. Four infants (23,5%) were diagnosed as Wolf-Parkinson White Syndrome and 4 (23,5%) had a history of fetal SVT. Seven infants (41,2%) responded to adenosin treatment, whereas 5 infants (29,4%) required combined medical treatment to turn back to normal sinus rythm. Five infants (29,4%) unresponsive to medical treatment required synchronized cardioversion. Four infants (23,5%) were found to have an underlying congenital heart disease. Conclusion: Neoanatal SVT can manifest antenatally, present with nonspecific signs and symptoms and show a severe clinical course. Therefore, both prenatal and postnatal early diagnosis and immediate treatment is important in terms of prognosis.

Yenidoğan Döneminde Supraventriküler Taşikardi: Tanı, Tedavi ve Prognoza Etki Eden Faktörler

Amaç: Yenidoğanlarda en sık karşılaşılan semptomatik taşiaritmi supraventriküler taşikardi (SVT)’dir. Fetal dönemde başlayabilmesi ve ağır klinik seyri nedeniyle hızlı tanı ve tedavi yaklaşımı önemlidir. Gereç ve Yöntem: Çalışmamızda, Eylül 2009-Ağustos 2013 tarihleri arasında hastanemiz Yenidoğan Yoğun Bakım Ünitesinde SVT tanısı alan yenidoğanların klinik ve laboratuvar bulguları, eşlik eden hastalıkları, uygulanan tedavi yaklaşımları, tedaviye yanıtlar ve yanıtı etkileyen faktörler retrospektif olarak değerlendirildi. Bulgular: SVT tanısı alan 17 hastanın median yaşı 16 saat (2-625) olup, 9 (%52,9)’unda başvuruda kalp yetmezliği bulguları vardı. Dört (%23,5) hastada Wolf-ParkinsonWhite sendromu, 4 (%23,5) hastada fetal SVT öyküsü olduğu görüldü. Adenozin tedavisine yanıt 7 (%41,2) hastada görülürken, 5 (%29,4) hastanın kombine ilaç tedavisi ile sinüs ritmine döndüğü görüldü. Medikal tedavilere yanıtsız 5 (%29,4) hastaya senkronize kardiyoversiyon yapılması gerekti. Dört (%23,5) hastada ise konjenital kalp hastalığı saptandı. Sonuç: Antenatal dönemde başlangıç gösterebilmesi, nonspesifik semptomları olması ve yenidoğan döneminde ağır klinik seyri nedeniyle SVT’li hastaların prenatal ve postnatal dönemde erken tanı ve tedavisi, hastalığın prognozu açısından önemlidir.
